D2 Form

Does anyone have a copy of the D2 application form, that they can scan or pdf and send me ? I've looked online but its only available by ordering (7-10 days!) Theres no good reason for DVLA to do that when they quite happily let you download the D4. Bureaucracy at its best...

If no one can help, when mines eventually arrives I'll pdf it and put it up somewhere if thats ok with Matt..

IIRC, the D2 has a clear plastic sticky bit for you to stick a photo onto, so a scanned copy wouldn't help. The D4 is just normal paper.

Hmm.. I think that only applies if its your first licence, If your enclosing your full C/C1 licence theyre just adding a category. So no photo necessary.. That is, if you already have a photocard licence.

"If you hold a valid full photocard driving licence and want to apply for a provisional driving licence to drive larger vehicles, minibuses and buses all that is needed is the completion of the application form D2 and the medical report form D4. You must also enclose your photocard driving licence (with the D740 paper counterpart) to DVLA, Swansea, SA99 1BR.
